MISSION STATEMENT

The Mission of Johnson Children’s Services Inc. is to promote the growth of children and youth while strengthening the family unit. Johnson Children’s Services was organized to address the growing deficit of services to troubled children and families in our community. Every individual has the intrinsic right to have his or her basic needs met in a respectful, supportive manner. Families deserve every reasonable opportunity to preserve their family unit. We recognize that the family is the foundation of our community. We believe that children living in nurturing, stable environments will mature into healthy, productive members of society. Our goal is to provide community-based services to children and families for the purpose of strengthening the family unit, emancipating prepared, productive young adults and providing specialized, quality foster care. By meeting these needs in a caring, comprehensive and professional manner we will impact the quality of life for the children and families in our community.

 Program Description

 A new mixed modality model is being proposed in this document to provide a short term Admission / Assessment Foster Care program where children can be assess close to their home community. This program will be located in the Simcoe region close to the child’s family and community resources.

Location:
Single family home in Simcoe Region.

Staffing:
Parent model homes (both parents are full time) with Child and Youth Workers on shift & in the classroom.

Capacity:
4 beds in one home

Definition of Proposed Service

The New Foster Program will provide residential care for 4 youths from Simcoe County Children’s Aid Society for the purpose of Assessment and Referral to longer-term care or return home with community support.  These beds will be funded on a fee for service or contact basis.
The program will be structure See Appendix “A”. Youth will participate in a behaviour modification program that deals with personal and interpersonal performance and the positive re-enforcement of appropriate behaviours through recognition and reward. Consequence will only be secondary means needed to ensure compliance with safety and security requirements.
A wrap around service will work in conjunction with residential care but will include a more individualized treatment through a Community base agency or Psychological Support Services if deemed necessary. The primary wrap around support for Assess and Refer youth who will be relatively short term up to 8 weeks will be on assessment for purpose of identify a comprehensive long term treatment plan for the youth either in the community or referral to a long term care facility.

 Intent of Service
Johnson’s Children Services intends by this service to be more pro-active and collaborative in the delivery of this service. Residential service alone does not adequately address the needs/ issues related to the family and community as it relates to the youth in care. By this service, Johnson’s Children Services will endeavour with the cooperation of one or several community based agencies to bring about a more holistic and effective service that will bring about desirable changes in both the child in care as well as the families

 Objectives of Service

To provide for the basic physical needs of the youth in care including food, clothing, shelter and recreation during the period of assessment
To assess and address the emotional needs of the youth during the period of assessment through individual counselling both within the program or at a community resource
To provide support to the family during the period of assessment strategies in restoring their relationship and upon request in some cases assistance in accessing community base agency to assist them further in exercising their roles as parents more effectively.

 Clinical Support Services
Definition of Service
Johnson’s Children Services in collaboration with a community resource will assess and provide assessment, short term counselling and referral to community resources that may be deemed appropriate to address the needs of the youth and/or the family.

Clinical Consultation

The role of the psychologist will be:

Assessment of youth

Consultation with referring agency and family

Assist in referral to community base agency

 

Wraparound Service

 

The role will:

Provide assessment and short term counselling

Referral to community base agency for long-term support

 

 

 

 

 

Intent of Service

 

Johnson’s Children Services together with community resources intend to provide a comprehensive assessment identifying the issues that resulted in out of home placement and recommend best course of action to address the issues in a short and long term treatment plan.

 

Objective of Service

 

To provide the youth with individual counselling to identify the issues that precipitated the separation from home

 

To provide support to the family by assisting them in accessing community resources.

 

Appendix “C”

 

Special Education

 

This service common referred to, as Section 23 classroom will operate onsite.

 

It is a special education program under the auspices of the Ministry of Education and Training and supervised by the area School Board. The focus of such program is academics with a treatment component consisting of Personal Life Management, Life Skills.

 

The program will be staffed with a qualified teacher, a school liaison worker and a Youth Worker and is under the Supervision of the Program Manager.

 

The team will meet bi-monthly to Asses the youth academic grade level and behaviour in the classroom. Recommendation will be made either to re-connect with his community school or another day treatment facility as the need dictates.

 

Intent of Service

 

The youth in this classroom are either unable to attend their home school due to geographic barriers or their behaviour warranting a specialized classroom. This is a short-term program not an alternative to community school.

 

Objectives of Service

 

To establish a classroom with a positive environment that meets the individual needs of the youth

 

 

Outcome:

The classroom will be the centre that focuses on literacy and numeracy but in a creative non-traditional teaching method.

 

 

 

 

 

 

To assist youth in developing behavioural, social and organizational skills needed to cope with academic demands through regular individual counselling and groups

 

Outcome:

Student will develop skills and have support necessary to minimize the effect of their behaviour on academic performance.

 

To provide periodic reports on students academic standings

 

Outcome

Referral agents will have the information necessary to plan for future education placements.
